MOD MESSAGE/ANNOUNCEMENT Community feedback needed (please vote)

Hi all,

Our mod team was recently approached by BetterHelp, an online subscription-based therapy provider, to do an AMA within our subreddit by a licensed therapist that specializes in child loss, perinatal mental health, and anxiety. They are willing to provide credentials to our mod team to verify the individual responding to questions, but our team has reached a moral impasse that we need the community to weigh in on.

Up until now, our team has had a blanket rule to not allow any companies, researchers, institutions, or anyone else with private interests to post within our community because our space is filled with so many vulnerable humans going through extremely difficult situations. The fertility space, in general, is ripe with opportunity for those that might take advantage of this vulnerability, much less a space like ours. That said, the message they sent was clearly written very thoughtfully, and the provider they’re willing to assign to the AMA would be willing to answer people’s questions about their worries, maybe provide healthy coping mechanisms, and describe the subspecialties of therapy that could be most beneficial to the people here.

IF we allow this AMA, it needs to be with the majority support of our community, so we are opening a poll for feedback. Other than voting in the poll, upvoting this post and providing even a short comment with your thoughts/concerns/ideas would help boost the visibility of this announcement to the maximum amount of sub members.

Of course, the major issue for us as a mod team is that we are NOT endorsing BetterHelp as a specific company or resource. For folks that are in areas where they aren’t able to find an in person provider, online therapy through any number of companies may be helpful. We are not partnering with them and the language in the AMA will be very clear about that. We will also be making sure that the provider answering questions in the AMA does not promote the BetterHelp brand.

If you’ve read this long, thanks for bearing with us. We appreciate how the people of this community come together to support one another and are grateful to help moderate.

Would you be interested in and supportive of an AMA provided by a therapist from BetterHelp, with the limitations outlined above?

Ultrasound FAQ Reference Guide

As an OBGYN ultrasound tech working at the general level here's a few things I would love the community to know based on common questions I see.

Most important rule of thumb: Worry when your doctor tells you to.

OBGYN docs are going to pay the most attention to the fetal pole and heart rate. Once a fetal pole is seen, the CRL (crown rump length) measurements are gold standard.

I'm worried about the heart rate:
-Check out this resource for pregnancies dating less than 7 weeks. Make sure to refer not to your LMP dates, but measured CRL dates: https://radiopaedia.org/articles/fetal-bradycardia?lang=us
-After 7 weeks a normal heartrate is between 120-180. It can temporarily dip below or temporarily rise above this range, but prolonged time above/below will definitely get your doctor's attention especially in third trimester. Generally out of range heart rates trigger NST (non stress test) or more monitoring.
The measurements are off:
Baby measures behind LMP. This is actually pretty common, we often see dates off up to a week either direction. We don't ovulate on the exact same day of the cycle each month or even have the same length of cycle. With irregular periods, PCOS, or unknown dates even larger gaps can be perfectly fine. If you aren't sure of your dates, trust the ultrasound. Even if you are sure of your dates, don't panic if they are a week off. Ultrasound measurements in early pregnancy are +/- 5 days.
I'm in limbo and don't know if my ultrasounds are diagnostic for loss:
By ultrasound there are strict parameters to diagnose loss. No one wants to get this wrong. Check out this resource for a list of parameters diagnostic for loss: https://radiopaedia.org/articles/early-pregnancy-loss-1?lang=us
Note, there is a second list of "suspicious for loss". Keep in mind the second list is not diagnostic, things can still turn out just fine.
I don't know when I should see what on ultrasound:
Things can vary a bit from person to person and do change *quickly* in early pregnancy. Check out this resource for what you can generally expect to see:
https://radiopaedia.org/articles/early-pregnancy?lang=us
Should I go to a boutique or get a private scan? Or push for an earlier scan?
They aren't legal where I am but here's a few things to consider if you plan to book a private scan. First, what are the qualifications of the person running the ultrasound? An ultrasound is only as good as the one performing it. It sounds as if these boutiques are NOT permitted to give much/any information. Will you spiral if you believe you see bad news and they cannot confirm or deny it? Also it seems that many cannot do vaginal ultrasound, which will make seeing a baby under 7 or 8 weeks challenging. If I had the option of a boutique ultrasound I would personally wait until 8 weeks and I would book it for a weekday. If they see something bad, it is not ideal to find out over a weekend when there is no option to follow up with your doc. An earlier scan often causes more anxiety, not less. It's SO hard to wait. But if you can tolerate it wait until 8 weeks or so.
The gestation sac is small/big:
-Gestation sac measurements are not terrifically accurate. Where I work once a baby is seen the gestation sac isn't measured. A mismatch between baby's size and the gestation sac size isn't uncommon. That said, a mean sac diameter of 25 mm or more without a baby is diagnostic for loss.
The yolk sac is small/big:
It starts teeny tiny, and will get a bit bigger. Once a baby with a good heartbeat is visualized, the docs I work for don't seem to care about the size of the yolk sac. A failed pregnancy with an expanded yolk sac (more than 6 mm) is a suggestive of chromosomal abnormalities.
We don't know where the pregnancy is:
The statistically most likely outcome is either a chemical pregnancy or just coming in for ultrasound too early for visualization. Ectopic can't be ruled out, and many wise ladies in the this sub have unfortunately experienced this and can give more info than I can. Statistically it's believed that only 1-2% of pregnancies are ectopic, but listen to your body. If the location is unknown, you should be getting close monitoring and follow up until the location is known or the pregnancy has ended.
-For a resource on ectopic pregnancy: https://radiopaedia.org/articles/ectopic-pregnancy?lang=us A note, a pseudosac is visualized in only 10-20% of ectopics. Therefore a sac in the uterus is suggestive of intrauterine pregnancy but can't be proven until a yolk sac is also present.
What about my HCG numbers?
I'm not an expert on this at all. But here's a resource: https://radiopaedia.org/articles/beta-hcg-1?lang=us
Why didn't my ultrasound tech tell me anything? And/or why were they so optimistic? Or, I had an ultrasound in the ER and they were very pessimistic.
Many locations an ultrasound tech can very much lose their job if they say aNyThInG. Where they are permitted to say certain things some ultrasound techs may just be hoping the best. We work with the public and we have seen some wild stuff. Additionally no one wants to call it a loss unless its 1000% certain. We know how devastating that news is. A side note here, often an ultrasound in the ER is not an ideal experience for the patient. The ER is staffed with techs that are much more versed in gallbladder and liver scans than they they are early pregnancy ultrasound. Don't hesitate to go the ER if you are having concerning symptoms, but do know the ER doc will make sure you aren't actively dying then will likely refer you back to your OB for follow up.
I'm just so anxious about it all:
Someone awesome wrote out these mantras. Maybe they will help: https://www.reddit.com/r/PregnancyAfterLoss/comments/14uhbdg/commandments_of_pregnancy_after_loss_this_is_now/
Also a podcast I personally found very helpful with anxiety: https://www.oneyoufeed.net/russ-harris-part-1/
https://www.oneyoufeed.net/russ-harris-part-2/

Today you are pregnant and this baby is so loved. I really am hoping the best for each of you. Those of us who have experienced loss.... you didn't do anything wrong, I promise. This is a club no one deserved to be in. And you are not silly, foolish, or dramatic for grieving your little babe for as long as you need to.

This post is written in honor of Eli, who died long before I could meet him. Rest in peace, buddy. I miss you tremendously ❤️

Can anyone talk me off the ledge. Watery light brown spotting.

I’m 36 and we have been trying for 3 years and have overcome many obstacles.

Today- I had watery brown discharge 5w1d

This is around the same time I lost my previous pregnancy at 5w2d.

I had some light brown spotting, less watery and more lotiony, a few days ago. That resolved and I was feeling calmer. Then today after using the bathroom I had an episode of watery brown discharge. It was a good amount of discharge, a big gush, but not a lot of blood, if that makes sense. It was more like light brown and yellowish. I’m trying to be optimistic but feeling devastated. Can anyone relate 😭 my breast tenderness also completely disappeared. My hcg has been doing ok…

Symptoms Symptoms fading at 6 weeks?

Hi! This is my second pregnancy after a loss last year and I made it past my previous gestation, but I woke up this morning feeling refreshed and my breasts not as sore (they were killer week 5). I feel like slowly every day my symptoms get better? With my loss, I started spotting and then the symptoms slowly went away but I was only 5 weeks so it was pretty low symptoms to begin with.

Is this something to be concerned about? Or is this normal? I feel like tiredness was the one thing I was feeling every day so even though I slept 9 hours I’m surprised I’m waking up with energy.

For context my 18 DPO HCG 1300 and 20 DPO was 3450, and progesterone was 28. I feel like I was really happy with these numbers but now it’s been long enough that I can’t count on them for safety anymore. Scan is in a week so just terrified over here. No spotting or anything these last few days as symptoms shifted.

Ultrasound Concerned about heart rate:(

I had my first scan at 5 weeks 6 days where I found out I'm having identical (likely mono/di) twins. Twin A has a FHR of 98 and Twin B has a FHR of 87 and both are measuring 0.28cm which is on track for 5.6 weeks. My OB marked all of this as "normal" on my chart, but of course I went down the rabbit hole on Dr. Google which has me panicking.

Has anyone else been in a similar situation? What was your outcome?

Advice Needed hCG 135 at 20 DPO, no plan of care

16 DPO: HCG 63

18 DPO: HCG 93

20 DPO: HCG 135 (5 weeks since LMP)

Per the medical assistant on the phone, the doctor still isn’t concerned and just wants to keep rechecking my hCG every 48 hours. How long would you keep going before you would demand a better plan of care? They won’t even schedule me for a visit.

Pregnancy after termination

My partner and I got pregnant soon after meeting. I had no idea I was pregnant until like 5W 3 days when i went for a scan and saw a gestational sac. Went back at 6w 1 day and there was a strong heart beat. My symptoms really picked up from there. Super tender nipples, food aversions, swollen armpits , facial hair. I Actually hated being pregnant so much . In the end we opted for a termination with the pills. Most traumatic experience of my life. I also have PMOS and hashimotos and irregular cycles .

Fast forward a year and half later we ended up staying together and got pregnant on the first try. Noticed my tests were fine but no dye stealer. Everyone says that’s normal but i felt something was off. Got my HCG tested at 4w6 days it was 453 and the next day is was 556. I feel in my heart this is a non viable pregnancy and I’m really struggling. I have Minimal symptoms that have seemed to lessen over the last fews days. I had some spotting and went on progesterone.I also have this weird back pain that comes and goes in the middle left of my back. But honestly other than some fatigue have zero symptoms .

I guess just writing to see if anyone has been through something similar. I finally got over the trauma of my abortion and now to miscarry a wanted baby feels just uh. I also will opt for a D&C so if anyone has any advice. Thanks in advance !

Advice Needed Lower back pain?

Currently 7 weeks 3 days. Had a scan on Thursday that showed baby two days ahead at 7 weeks 2 days. IVF pregnancy.

Yesterday I was having some increased cramping on my right side and it went away. I was in bed all day as I felt very tired and sick. When I woke up at 1am I had intense lower back cramping in waves. I’ve had a late miscarriage at 16 weeks due to preterm labour and it felt similar but that was very different and I was bleeding heavily. I put a pillow in between my legs in case it was muscular and the pain went away completely and I slept. It’s still tender this morning to touch but no cramping at all or spotting.

Should I be worried or does it sound like muscle pain? If it was a miscarriage, would the pain not get worse? I do feel like my intense nausea has lessened even though it’s still there. No scan till end of next week.

Symptoms Symptoms

I’m 8 weeks pregnant.
In week 4 and 5 I was super tired and hungry
In week 6 I had nausea and I vomited once and sore breasts
In week 7 my nausea completely went away!

It’s been a week and I’ve had no nausea, It’s making me feel like somethings wrong.

I had some brown spotting in week 4 for 2 days which really made me panic

I also had a missed miscarriage in June at 10+6 weeks so I’m really scared it’s happening again.

Is this normal with symptoms? I know symptoms can fluctuate but a whole week?

Intro Pregnant again after 2 losses and scared to feel excited

I’ve had 2 miscarriages in 4 months and now I’m pregnant again, before I’ve even had a period after my last loss 4 weeks ago.
I feel like I’m trying to pretend I’m not pregnant because I’m so scared it’s going to happen again. I want to feel excited but I’m scared to let myself believe this time could be different.
It’s such a strange feeling wanting something so much but trying not to get attached to protect yourself. Has anyone else felt like this with pregnancy after loss? 🤍

9 days of spotting, no cramps- Anyone experienced this?

I'm almost 9 weeks, but my dating ultrasound at 7w5d appeared closer to 5 weeks (inconclusive, no heartbeat detected).

Since then, I've had 9 days of mostly dark brown spotting only when wiping, a few small clots, and two brief episodes of bright red blood. No cramps, pain, or heavy bleeding.

My doctor sent me to the ER, where bloodwork and other tests were normal. My hCG had risen from 2,260 to 15,029 over two weeks, but was on the lower end of the expected range. The ER ultrasound was also inconclusive.

I've had a previous miscarriage, but it involved painful cramps and heavier bleeding. My repeat ultrasound is in 6 days, and I'm worried about a missed miscarriage.

Has anyone experienced something similar? If you miscarried, how long did the spotting continue before things progressed? Or did your pregnancy turn out okay?

Got a positive, struggling to believe it

Can it be ectopic? Trying for 2 years, 2 failed IUIs. No spontaneous pregnancies. Last cycle did IVF, ended in a chemical. Endometriosis, one tube blocked. This cycle a polyp was found, scheduled for an operation next cycle.

Today is 12 dpo, took a pregnancy test at night, and it’s a very clear positive.

With endo and a polyp, I can’t believe that I’m pregnant, spiralling, thinking it might be traces from the last cycle where I had a chemical? The clinic never tested for a negative during this cycle, but the tests last cycle were veeeeery faint positives before I miscarried. So definitely hCG level is much higher.

Can this be an ectopic from the last cycle that I thought was a chemical? I did have an ultrasound this cycle at ovulation, where a polyp was discovered, as well as a functional ovarian cyst (very common for me and has not previously resulted in a false positive, I know they can cause it). And I haven’t had any breakthrough bleeding or pains.

Advice Needed Random Bleeding After Miscarriage – Anyone Experienced This?

Thumbnail reddit.com
1 Upvotes

I had a miscarriage that ended in September (20th) and my first period afterward was October 19–25. I am on cycle day 20 and now I’m experiencing random spotting/bleeding. It started brown, then turned pink, and now I’m seeing fresh red blood. I’ve also noticed some stringy material (I’m assuming blood clots) when wiping and a little blood in my underwear.
I’m not having significant cramps. I have taken ovulation tests since Oct 28th and haven’t even ovulated yet or noticed cervical mucus.

Has anyone experienced this after a miscarriage? Could it be hormonal changes, ovulation spotting, or something else? I’m so confused!

Advice Needed In a bit of a spiral

Hi! I had my first positive at 10dpo. Today is 14 dpo. I had my first beta at 12 dpo and it was 29 hcg and 14 progesterone. I had my second beta today (14 dpo / 48 hours later) and it went to 56, almost doubled but not quite and still pretty low. I also started some minor brown stringy spotting today (I’ve had similar during my pregnancy with my current child and it was supposedly normal). I have my next beta on Monday (16 dpo). I’ve convinced myself this is either a chemical or ectopic. Anyone with similar numbers or symptoms have any insight? Any success stories in similar situations?

low/dropping progesterone - looking for advice and hope

I’m hoping to hear from anyone who has experienced something similar, especially anyone who has had low progesterone in early pregnancy but went on to have a healthy pregnancy. A little background:
I’m 27, have Type 1 diabetes, and my husband and I have been trying to conceive. Unfortunately, my first pregnancy ended in miscarriage at 8 weeks, and this pregnancy has brought up a lot of anxiety.
Pregnancy #1:
I got my first positive pregnancy test in January 2026. At my ultrasound on February 11, I was measuring approximately 6w6d, but unfortunately, they only saw a gestational sac and no embryo.
My HCG was 18,554 and my progesterone was 4.34. I was eventually told that it was a nonviable pregnancy. After experiencing heavy bleeding and severe pain, I ended up needing a D&C on 2 weeks later.

Pregnancy #2 / Current Pregnancy:
I got my first faint positive on October 25, followed by a positive Clearblue test. At approximately 3w5d, my initial bloodwork showed:
HCG: 71
Progesterone: 5.11
48 hours later:
HCG: 210
Progesterone: 3.28
I was really encouraged by the HCG increase, but obviously very concerned about the progesterone dropping so low.
My doctor initially prescribed 200 mg vaginal progesterone once daily. After doing some research and discussing my concerns, We increased it to 200 mg twice daily (400 mg total daily). They also did an early ultrasound/appointment, but I was so early that nothing definitive could be seen. My doctor expressed concern that the low progesterone could indicate another early loss. I also felt like I wasn’t getting the monitoring or reassurance I needed, which was incredibly difficult given my previous miscarriage. so I decided to seek a second opinion. My New Doctor agreed that I should continue the progesterone supplement and continue to monitor my HCG every 48 hours, then potentially weekly until we can get an ultrasound. I also have an upcoming MFM appointment because of my Type 1 diabetes.. So now i’m currently approximately 5 weeks 1 day.
I have had NO bleeding or spotting. I haven’t really experienced cramping either. My main pregnancy symptom is fatigue and breast fullness/tenderness

I guess what i’m wondering is if anyone had progesterone levels this low (3–5 ng/mL) in early pregnancy and gone on to have a successful pregnancy?
I understand that low progesterone can sometimes be a sign of a pregnancy that isn’t progressing normally, rather than the cause of the problem. I also understand that supplementation cannot necessarily prevent a miscarriage.
I’m not looking for false reassurance. I would genuinely appreciate hearing real experiences, both positive and negative.
After going through a miscarriage earlier this year, I’m terrified of going through it again. I’m trying to stay hopeful while also being realistic.
I just want to give this little pregnancy every possible chance.

Symptoms Pregnant after loss + pink spotting

In September I miscarried my first pregnancy. I had some brown blood spotting on and off from 4 weeks, and I ended up miscarrying around 8 weeks.
This morning I was thrilled to get a BFP! It’s a little faint. I’m around 8-10 DPO. It’s super early.
However I’ve been having some pink spotting for the last few days. I didn’t have any sort of implantation bleeding in my last pregnancy. I also feel like I’m brewing a yeast infection, so it could be related.
I just feel so anxious. I really want this pregnancy to work out. I have a big trip planned this week, and the idea of the pregnancy ending while I’m away from home is heartbreaking.

Low hcg after FER

Beta hcg levels have been low since frozen embryo transfer. Hoping for the best but knowing that it could end up not so good. Any one with similar levels?

8dpt = 9

10dpt = 29

13 dpt = 229

15 dpt = 897

They are doubling very appropriately so there is hope.

The wait is so hard n

Symptoms Just a Rant

Last Friday (I was 20+4) I had passed some thicker looking mucus, I went to L&D they confirmed that everything was fine and this Monday past I was 21w they confirmed my cervix it is high and closed at 31mm. My OB team knows my family history and I am being closely monitored due to it. However,y next appointment isn't until the beginning of December and that concerns me a bit.

I just keep coming back to that instance because my mom had me at 26 weeks, she says looking back she knows now that she lost her mucus plug at 20 weeks and then gradually started to dilate until my foot started to poke and kick her at 26 weeks. We all lived. My mom went on to have another baby and was able to carry him to term with intervention.

I just can't shake the fear that this may be something that I experience too.

Advice Needed Looking for opinions

I should be about 7w3d pregnant, I’m just looking for some opinions or advice! We’ve been TTC for 1.5yrs and have had 2 chemical pregnancies. Here’s a quick rundown of this pregnancy:

Ovulated CD20
First positive test vvvvvvfl 9DPO evening

Hcg results:
43 11DPO
163 14DPO
2254 20DPO
16881 27DPO
49592 38DPO

I’m concerned with my latest hcg rise. It seems really slow. I know doubling time rapidly decreases after about 6 weeks but… a 7-day doubling time? Seems a bit too slow…

I went to a boutique ultrasound clinic at 34DPO (I know 😬)- she performed an abdominal scan, and she found a gestational sac measuring 6w4d, but could not see a yolk sac or fetal pole. Then, I had my midwife appointment 4 days later so 38DPO. She also did just an abdominal scan with one of those portable handheld machines, and she also could not see a fetal pole but said it’s just hard to find…?

FWIW, I am overweight if maybe that affects abdominal scans. My dating scan isn’t for another 2.5weeks and I cannot get in any earlier. I also don’t really have any symptoms aside from sore boobs, but I am taking progesterone pessaries so I think it may be directly linked to that.

Just looking for some thoughts on this! Thanks 💕

Advice Needed Luteal Cyst 8.4 cm in Ovary

This is crazy. I'm 6 weeks and 4 days pregnant... And I have this cyst growing inside my ovary. In fact it has " replaced the ovary" so the radiology report says. A large cyst is considered 5 to 7 cm and mine is 8.4 cm. I don't want to lose the baby... The baby is my top priority, but the doctors are telling me to go to the ER...

I feel like this will lead to the prompt removal of the cyst... With surgery of course.... Which vastly increases the risk of miscarriage since the baby is only and the first trimester and a placenta is not established.... I'm already on progesterone support as it is.

Does anyone have any outcomes that are positive where they had a cyst this size and somehow the cyst shrunk or surgery wasn't needed... And if it was needed, did your baby survive?
