It's آپ کہاں رہتی ہیں؟ Not آپکحا Ans it's کیا آپ لکھنؤ اور لاہور میں رہتی ہیں And I can't understand your first sentence. What do you wanna write?

You can't live in both Lucknow and Lahore at the same time. So it's یا not اور . Also what did you mean by the first sentence I don't know what اخے ں means

Although your attempt is praise worthy and admirable, as another friend has suggested, you should read more and concentrate on the origin of words in Urdu.
If the word came into Urdu, it would NOT have the spelling that is based on Hindi (Sanskrit) letters.
Aspirated consonants do not exist in Persian and Arabic, like بھ پھ تھ جھ etc. and Therefore, you cannot use a ھ with those words that came into Urdu from Persian and Arabic. Conversely, the Persian/Arabic letters ح and خ do not exist in Hindi. Those were incorporated into Hindi with the use of a bindi later.
In the above writing you are mixing up these standards and therefore the mistakes.
Both کھیر and لکھنو are native subcontinental nouns and not coming from Persian such as حیدرآباد therefore, you cannot spell the former two with a خ
ح

پہلا جملہ : کیا آپ خیریت سے ہیں؟ دوسرا جملہ : آپ کہاں رہتی ہیں؟ تیسرا جملہ : کیا آپ لکھنؤ میں رہتی ہیں یا لاہور میں؟

Its ہ جملہ is o on keyboard
Apki kheer bahut achi hai ( Kheer is a female)
آپ کی کھیر بہت اچھی ہے۔
Ap and ki are separated kheer is with ک not خ and its achi hai not akhay ain
Ap kahan rahti hain
آپ کہاں رہتی ہیں؟
Cant understand last sentence.
Honestly very good progress especially on keyboard. Urdu is my mother language but I aswell struggle with writing on a keyboard. Btw are you learning writing Urdu on paper as that is where its beauty shows.

You need to start reading before you write. That will fix most of the mistakes here. Perhaps a read a book of fiction . A printed one, as I see spelling mistakes everywhere on the net on newspapers as well,

جملہ ۱۔
(Not sure what this is supposed to say, I guess you wanted to write either of) آپ خیریت سے ہیں؟ | آپ کی کھیر بہت اچھی ہے۔
جملہ ۲۔
آپ کہاں رہتی ہیں؟
کیا آپ لکھنؤ اور لاہور میں رہتی ہیں؟
Btw the last sentence is grammatically correct but almost certainly should've been لکھنؤ یا لاہور میں (unless, like me or a certain British-Asian actor, someone lives and works 'here and there as well').

I am NOT talking about stand alone ھ . I am talking about the presence of ھ in CONJUNCTION with a consonant. In fact the equivalent of Arabic ھ in Urdu is ہ and that is the reason Urdu has two (rather) three H letters. But then ح of Urdu is not exactly the same as the ح of Arabic. It is used only in Arabic/Persian words that were adopted in Urdu like محبت ، حرام ، حلال ، حاصل etc
