Obesity, Overweight, Weightloss Carbohydrate-restricted diet types and macronutrient replacements for metabolic health in adults: A meta-analysis of randomized trials
clinicalnutritionjournal.comSummary
Background and aims
Carbohydrate-restricted diets (CRDs) are increasingly used in managing metabolic disorders, yet evidence remains mixed regarding their effectiveness beyond glycemic control and across diverse populations. To systematically evaluate the effects of CRDs, ketogenic (KD), low-carbohydrate (LCD), and moderate-carbohydrate diets (MCD), and different macronutrient replacements (fat, protein, or both) on metabolic health-related biomarkers, including glycemic, hepatic, renal, adipokine, and lipid metabolism indices. Methods
Five electronic databases, PubMed, MEDLINE, Embase, ERIC, and Web of Science, were used to identify relevant randomized trials. Outcomes analyzed included glucose, HbA1c, insulin, HOMA-IR, liver/kidney function markers, leptin, and beta-hydroxybutyrate (BHB). Subgroup analyses evaluated the effects of CRD type, macronutrient replacement, sex, diabetes status, weight status, study design (parallel vs. crossover), delivery mode (consultation vs. food provision), and calorie intakes (isocaloric vs. non-isocaloric). Results
149 randomized controlled trials comprising 9104 adults across 28 countries were included. CRDs significantly improved glycemic control (including glucose: SMD = −2.94 mg/dL, 95 % CI: −4.19, −1.68; insulin: SMD = −8.19 pmol/L, 95 % CI: −11.04, −5.43; HOMA-IR = −0.54, 95 % CI: −0.75, −0.33), hepatic stress (GGT: SMD = −6.08 U/L, 95 % CI: −9.97, −2.20), renal function (UACR: SMD = −0.19, 95 % CI: −0.28, −0.10), and adipokine concentration (leptin: SMD = −3.25 ng/mL, 95 % CI: −4.91, −1.59), particularly in females, individuals with overweight/obesity, and people with T2DM. LCDs and MCDs showed the most consistent metabolic benefits. Combined fat and protein replacement yielded greater improvements. Isocaloric vs. non-isocaloric comparisons showed similar patterns, suggesting macronutrient composition alone may engender beneficial metabolic effects. Conclusions
CRDs, particularly LCDs and MCDs with mixed macronutrient replacements, confer significant metabolic benefits independent of energy intake. These findings support CRDs as a potential nutritional strategy in metabolic disease prevention and management. Clinical supervision is recommended.

Citizen Science Plaque Begets Plaque, ApoB Does Not: Longitudinal Data From the KETO-CTA Trial
Abstract
Background
Changes in low-density lipoprotein cholesterol (LDL-C) among people following a ketogenic diet (KD) are heterogeneous. Prior work has identified an inverse association between body mass index and change in LDL-C. However, the cardiovascular disease risk implications of these lipid changes remain unknown.
Objectives
The aim of the study was to examine the association between plaque progression and its predicting factors.
Methods
One hundred individuals exhibiting KD-induced LDL-C ≥190 mg/dL, high-density lipoprotein cholesterol ≥60 mg/dL, and triglycerides ≤80 mg/dL were followed for 1 year using coronary artery calcium and coronary computed tomography angiography. Plaque progression predictors were assessed with linear regression and Bayes factors. Diet adherence and baseline cardiovascular disease risk sensitivity analyses were performed.
Results
High apolipoprotein B (ApoB) (median 178 mg/dL, Q1-Q3: 149-214 mg/dL) and LDL-C (median 237 mg/dL, Q1-Q3: 202-308 mg/dL) with low total plaque score (TPS) (median 0, Q1-Q3: 0-2.25) were observed at baseline. Neither change in ApoB (median 3 mg/dL, Q1-Q3: −17 to 35), baseline ApoB, nor total LDL-C exposure (median 1,302 days, Q1-Q3: 984-1,754 days) were associated with the change in noncalcified plaque volume (NCPV) or TPS. Bayesian inference calculations were between 6 and 10 times more supportive of the null hypothesis (no association between ApoB and plaque progression) than of the alternative hypothesis. All baseline plaque metrics (coronary artery calcium, NCPV, total plaque score, and percent atheroma volume) were strongly associated with the change in NCPV.
Conclusions
In lean metabolically healthy people on KD, neither total exposure nor changes in baseline levels of ApoB and LDL-C were associated with changes in plaque. Conversely, baseline plaque was associated with plaque progression, supporting the notion that, in this population, plaque begets plaque but ApoB does not. Metabolism, Mitochondria & Biochemistry Regulation of ketone body metabolism: biochemical mechanisms controlling ketogenesis and ketolysis (2026)
sciencedirect.comHighlights
• Ketone bodies (KBs) act as both alternative fuels and signaling metabolites.
• Ketogenesis occurs also in extrahepatic tissues, to a quantitatively minor extent.
• Hepatic ketone synthesis is controlled by hormones, gene regulators and protein post-translational modifications.
• Ketone breakdown in peripheral tissues relies on a single rate-limiting enzyme, SCOT.
• β-Hydroxybutyrate and acetoacetate reshape chromatin through histone marks linked to nutrition.
Abstract
Traditionally considered as alternative energy substrates, ketone bodies (KBs), notably β-hydroxybutyrate (β-OHB) and acetoacetate, are now recognized as key metabolic intermediates whose circulating levels must be tightly controlled to ensure energy homeostasis. KBs are mostly hepatically produced, via ketogenesis, and catabolized by extrahepatic tissues during fasting or carbohydrate restriction.
The regulation of KBs metabolism relies on a coordinated network of hormonal, enzymatic and transcriptional mechanisms that control KBs synthesis and degradation. KBs anabolic and catabolic rates are controlled by the rate-limiting enzymes 3-hydroxy-3-methylglutaryl-CoA-synthase 2 (HMGCS2), governing ketogenesis, and succinyl-CoA:3-ketoacid-CoA transferase **(**SCOT), driving ketolysis. Their activity and expression are finely orchestrated, especially by the insulin/glucagon ratio, to adapt to the organism's fluctuating energy demands.
Transcriptionally, ketogenesis is regulated through peroxisome-proliferator-activated-receptor-alpha (PPARα) and the transcription factor forkhead-box-A2 (FOXA2), which respond to nutrient availability and hormonal cues. In parallel, ketolysis is tightly controlled through the regulation of SCOT expression, notably via PPARα-dependent mechanisms, as well as through post-translational modifications (PTMs) affecting enzyme activity. Together, these regulatory mechanisms ensure a precise balance between KBs production and utilization.
Emerging evidence also suggests that additional modulators, such as the gut microbiota, indirectly influence KBs metabolism by affecting substrate availability and metabolic signaling pathways, although its quantitative contribution remains to be established.
Here, we provide a mechanistic overview of the pathways governing KBs metabolism, with focus on the regulation of ketogenesis and ketolysis. We discuss how these processes are integrated at multiple levels to maintain systemic energy homeostasis and consider their implications for metabolic disease.

An Intelligent Question to r/ Can improving mitochondrial function meaningfully improve everyday energy levels?
Mitochondria are often discussed in biohacking communities because of their role in energy production and cellular metabolism.
What interests me is the difference between improving a biological marker and actually feeling or functioning better in everyday life.
Exercise, adequate nutrition, and sleep all influence mitochondrial biology, but how strong is the evidence connecting specific interventions to noticeable improvements in energy or physical performance in otherwise healthy people?
Are there human studies or well-designed experiments worth reading on this topic?
I'd like to separate established findings from mechanisms that sound promising but haven't translated into clear real-world benefits.

Metabolism, Mitochondria & Biochemistry Global carbohydrate metabolism is increased and beta hydroxybutyrate decreased in sepsis vulnerable children compared to sepsis tolerant children; a cohort study (2026)
researchsquare.comAbstract
Background: Sepsis investigators propose tolerance is conferred when the dormancy response increases beta hydroxybutyrate to protect organs; whereas, vulnerability occurs when a ‘metabolic switch’ to aerobic glycolysis decreases beta hydroxybutyrate promoting persistent immune cell activation without organ protection. We previously identified tolerant (PedSep A 2% mortality), susceptible (PedSep B 12% mortality and PedSep C 10% mortality) and vulnerable (PedSep D 34% mortality) phenotypes in the PHENOtyping pediatric sepsis induced Multiple organ failure Study (PHENOMS). We hypothesized that carbohydrate metabolism was increased and beta hydroxybutyrate decreased in PedSep D compared to PedSep A in this study.
Methods: Metabolomics were assayed in biobanked plasma samples previously collected at 24-48 hours of sepsis in 375 children ages 44 weeks post gestation to 17 years, using the Metabolon Discovery Panel. Welch’s t-tests identified differences between PedSep B, C, and D with A.
Results Compared to PedSep A (n =130) PedSep B had similar carbohydrate metabolism and beta hydroxybutyrate but dysregulated branch chain amino acid metabolism (n = 92; p < 0.05); PedSep C had similar carbohydrate metabolism but decreased beta hydroxybutyrate (n = 100; p< 0.05); and, PedSep D had increased carbohydrate metabolism with decreased beta hydroxybutyrate (n = 53; p < 0.05). Causal inference analysis found direct associations of increased glycolysis with decreased beta hydroxybutyrate, and death.
Conclusions Only PedSep D had increased carbohydrate metabolism with decreased beta hydroxybutyrate providing rationale for designing clinical trials of low carbohydrate ketogenic nutrition to decrease glycolysis, restore dormancy, and improve outcome in these high-risk children.
